Cryptomenorrhea refers to a condition where menstruation is obstructed despite the presence of menstrual blood, leading to hidden or undetected periods. There are two main types:
-
Primary Cryptomenorrhea: Occurs when a young woman has never menstruated, but menstrual blood is produced and accumulates behind an obstruction, such as a hymenal or vaginal imperforation, or a congenital malformation of the reproductive organs.
-
Secondary Cryptomenorrhea: Occurs in women who previously had regular periods but later experience an obstruction due to conditions like endometrial adhesions, cervical stenosis, or other anatomical issues.
Both types can cause discomfort, pain, and complications like infertility if left untreated. Diagnosis often involves imaging, physical exams, and sometimes surgical intervention to relieve the obstruction.
Cryptomenorrhea presents with signs and symptoms related to menstrual blood retention. Common symptoms include:
- Abdominal pain or cramping: Caused by blood accumulation behind an obstruction.
- Lack of visible menstruation: No external menstrual flow despite normal hormonal cycles.
- Pelvic fullness or discomfort: Due to retained blood in the reproductive tract.
- Painful menstruation: If blood is trapped, it can lead to increased pain.
- Amenorrhea: In primary cases, there may be no previous menstrual history.
- Difficulty conceiving: If left untreated, cryptomenorrhea can affect fertility.
Other signs can include back pain or bladder pressure, and an underlying obstruction often requires medical investigation for diagnosis and treatment.
